Join Dr. Alexandra Papaioannou with Dr. Amy Freedman and Lorna McDougall, co-leads of the Preventive Care for Older People (PCOP) tool developed through the Centre for Effective Practice. Dr. Freedman is a family physician and MD Lead for the St. Michael’s Home Visit Team, practicing at Baycrest and teaching at the University of Toronto. McDougall is a nurse practitioner and NP Lead for the Home Visit program for homebound older adults, with a clinical appointment at the University of Toronto’s School of Nursing.
